Family Support Team (FST)
The primary goal of the Family Support Team (FST) is to return children and adolescents home from residential or psychiatric placement and/or prevent residential and/or psychiatric placement through the use of intensive wrap around services. Program census capacity is 32.
Family Support Teams provide individualized treatment for children, youth and their families within the context of either a foster family or the biological family through the use of a 2 person team in collaboration with other providers. Treatment could include (but not limited to) individual and family therapy, case management, mentoring, recreational opportunities and medication consultation /management.
The target population includes children and adolescents who have recently been returned home from residential placement or who are at risk for residential placement in order to successfully stabilize maintain and/or reunify in their home community.
Area Served: The program serves the Bridgeport, Norwalk and Stamford service regions of the state Department of Children & Families (DCF). Length of Service: Approximately 12 months or until a minimum of 6 months stability.
