Transitional Care Coordinator

POSITION SUMMARY:  

 

1.      Provide support and assistance to promote youth success in the community.

2.      Participate in the new Admission- Orientation process.

3.      Participates in visitation with biological parent(s) and/or designee to assist with discharge transition and begin to build relationship with parent(s)

4.      Participate in the weekly Multidisciplinary Clinical Rounds Team meeting and once weekly Clinical Manager Team meeting for each youth on your caseload.

5.      Meet with the youth every other week and document any contact in a progress note by end of business day.

6.      Provide support to milieu staff in coordinating in-person visits, skype meetings, and telephone contacts with family and professionals on a weekly basis.

7.      Facilitate adolescent and family team meetings with the clinician once monthly post discharge either in-person or remote for the client’s length of stay.

8.      Serve as a role model and reinforce the adolescent’s individual treatment plan as well as continued support for the adolescent and family after completion of the program.

9.      Develop and provide a re-entry Care Plan packet within 10 days of admission to identify specific areas of need and thereafter at 30 days, 60 days. Within 60 days of discharge, develop the pre discharge plan to include re-engagement to next steps in the youth’s care plan and ensure a successful transition from the program to the community and home.

10.   Complete documentation as required in Carelogic electronic record.

11.   Ensure that any risk related treatment issues or client behaviors are discussed with Clinical Manager and Milieu Coordinator

12.   Facilitate adolescent and family team meetings with the clinician once monthly post discharge.

13.   Transport clients to and from medical appointments, dental appointment, and long-term substance maintenance clinic.

 

PURPOSE/FUNCTION OF THE PROGRAM:

The Kraft House is a 12-bed inpatient psychiatric residential treatment program for adolescent boys, including those boys with substance use disorder who have experienced emotional, physical, or sexual abuse. This program is a step down from inpatient hospitalization or a diversionary placement from a more restrictive environment. The program is designed to help adolescent boys return to home to their community or a long- term group- home setting.
