Client Grievances

Our staff is committed to working in partnership with children and families. On occasion, families may have questions or concerns about the services they are receiving, or how they are being treated. See Client Rights and Responsibilities in English and Spanish/Español.

Most of these matters can be resolved through discussion with the service provider or his/her supervisor.

However, if problems persist, or clients feel they are being mistreated, they have the right to initiate a formal complaint by completing the Grievance Form. This form is available at the link below in English and Spanish/Español or a copy can be requested in-person from a BGV staff member.

The completed form can be either emailed to BGVcompliance@bgvillage.org, mailed to the attention of the President/CEO or Compliance Officer at BGV 528 Wheelers Farm Rd. Milford, CT 06461, or left with a staff member to direct to the President/CEO or Compliance Officer. Clients may also submit a complaint to the State of Connecticut Commission on Human Rights and Opportunities at 25 Sigourney Street; Hartford, CT 06160.

Within two weeks of receiving the report, the President/CEO or designee will formally communicate the outcome to the complainant and his/her guardian as appropriate based upon actions such as interviews with staff member(s) and/or complainant.
